You will need the following information to complete your Payment Request:
• Social Security Number (first time only)
• Tele-Serv
Personal Identification Number (PIN) (first time only)
• Earnings Information (if applicable)
• Work Search Contact Log
Unemployment Insurance claimants:
KEEP YOUR TWC MAILING ADDRESS UP-TO-DATE! If you are eligible for unemployment benefits, you will receive a TWC UI debit card in the mail. You need this debit card to access your benefits. Tell us when your address changes, even if you filed a US Postal Service Change of Address form.
